63 year old David lives with friends in a Community-based home, attends Mize Community Learning center and sees 11 doctors from issues ranging from high blood pressure to diabetes. Managing his physical and mental health has been overwhelming, but has also increased his communication with the staff at Ray Graham Association & Medical Professionals to better coordinate his care.
